Critical Illness Insurance

1. As an active member in good-standing of the National Benefit Advisory Association, you are automatically covered under the Group Critical Illness insurance policy issued to the Association.
2. This policy provides the members enrolled spouse a maximum lump sum benefit of $2,500 each if such member and/or spouse is/are diagnosed with any of the following critical illnesses more than 90 days after that member's effective date unless otherwise ahown below.
3. Up to 100% of Critical Illness Diagnosis Benefits:

Life Threatening Cancer diagnosed more than 30 days after the member's effective date.

Heart Attack

Kidney (Renal) Failure

Stroke

Coma

Loss of Sight, Speech, or Hearing

Major Organ Transplant
